Dental Insurance:
As a specialty provider, we are contracted with several PPO insurance plans. As a courtesy to all our patients, we are happy to file the necessary claim forms and supporting documentation so that you receive the full benefits your policy offers. However, we make no guarantee of any estimated coverage. The insurance policy is an agreement between you and the insurance company, and so, the patients are responsible for all charges the dental insurance does not cover. If your insurance company has not paid their portion within 60 days from the date of service, you are responsible for the full payment. Please be sure to notify our office of any changes in your insurance coverage or personal contact information as soon as possible, as this may delay payments.
This is a list of the current insurance companies we are contracted with. If you do not see you insurance company listed, please call our office as we are continuously updating our provider status. If we are not a contracted provider with your insurance, we can still see you and bill your insurance, it will just be under out of network benefits.
PLEASE CALL OUR OFFICE WITH ANY INSURANCE QUESTIONS.
